	Print out the forms and have students and coach(s) complete them. Mail all forms with payment to the Minnesota Academy of Science (address below) Adult Medical Form (for coaches and students who will be 18 years old by the date of the tournament) Student Medical Form Parental Consent & Media Release Form ** The forms above are in pdf format. To fill out the form, open the file, place your cursor on a line and type in the information. The tab key will jump to the next line. When you are done, print out the page, add the signature and date, and mail the forms along with payment to the MN Academy of Science. If you close the file, your text will not be saved.

	In order for registration to be final, the on-line registration must be completed and all forms and payment received by the Minnesota Academy of Science by the deadline (High School event: 12/15/2009, Middle School event: 1/15/2010). A late registration fee of $50 per team will be assessed if the registration is not complete by the registration deadline.
